Diet, bowel habit, and lifestyle changes are often first-line therapy. When conservative therapy is not effective, in-office ligation, sclerotherapy, or infrared coagulation for hemorrhoids should be considered. Surgery is reserved for those with persistent symptoms or grade 4 disease.
